What the sound may mean
The sound is often clearer while breathing out, although it can occur while breathing in. It develops when air passes through narrowed or partly blocked breathing tubes. Asthma and COPD can present with wheezing; acute bronchitis may include cough and wheeze, while bronchiolitis can cause wheezing in babies and young children. A sudden allergic reaction can also present with wheezing, especially alongside facial or throat swelling, hives, or faintness.
One sound cannot identify the cause by itself. A clinician may consider when it began, whether it follows exercise, infection, smoke, dust, medicines, food, or an insect sting, and whether cough, fever, chest tightness, or breathlessness occurs with it.
What to do now
Sit upright, move away from smoke or another obvious airborne irritant, and loosen restrictive clothing. If you already have a prescribed reliever inhaler or nebuliser plan, follow its directions. Do not use another person's medicine or delay care while trying unprescribed remedies.
Arrange a medical assessment for a first episode, repeated episodes, or wheezing that does not settle. Note when the noise starts, what you were doing, possible exposures, associated symptoms, and whether prescribed treatment helped. A short audio recording may help if the sound stops before the appointment, but it should never delay urgent help.
When to seek urgent care
Call emergency services now for severe breathlessness, difficulty speaking, blue or grey lips, confusion, collapse, marked drowsiness, or swelling of the lips, tongue, face, or throat. Sudden wheezing after food, medicine, or an insect sting can be part of anaphylaxis and needs emergency treatment.
Seek prompt medical care if breathing is getting harder, the chest or neck muscles pull in with each breath, a prescribed reliever is not helping as expected, or a child may have inhaled a small object. A quieter wheeze does not always mean improvement if breathing effort, alertness, or skin colour is worsening.
