The short answer

Ovulation pain, also called mittelschmerz, can cause a one-sided ache or sharp sensation near the middle of a menstrual cycle. Severe pain should not simply be assumed to come from ovulation.

What the pain may mean

Typical ovulation discomfort often appears on one side of the lower abdomen and settles without ongoing symptoms. Pain can switch sides between cycles, but the timing alone cannot confirm its cause.

Marked or recurring pelvic pain may also occur with endometriosis or an ovarian cyst. Other pelvic, urinary, bowel, or pregnancy-related problems can feel similar, so a clinician may ask about cycle timing, bleeding, discharge, nausea, urinary symptoms, and the possibility of pregnancy.

Seek timely clinical advice when pain is becoming stronger, lasts beyond the expected mid-cycle episode, occurs at other points in the cycle, or comes with pain during sex, bowel movements, or urination.
