Hoarse Voice and Cough in a 6-Month-Old Baby

A viral cold can inflame the throat and voice box. A barking cough, harsh noise while breathing in, or symptoms that become more noticeable at night can occur when the upper airway is narrowed, as in croup. Because babies have small airways, a clinician should assess concerning or worsening symptoms promptly.

Hoarseness comes from irritation or swelling around the voice box. Notice whether the cough is dry, wet or bark-like and whether there is a high-pitched sound when the baby breathes in. Also watch the chest and neck for pulling in with each breath.

A blocked nose can make feeding noisier and trigger coughing as mucus reaches the throat. Coughing during feeds may instead relate to milk flowing too quickly or reflux. These observations help a clinician, but the sound alone cannot confirm the cause.

Keep the baby calm and upright while awake, because crying can make noisy breathing more obvious. Offer smaller, more frequent breast or bottle feeds and pause if breathing becomes difficult. Saline nose drops followed by gentle suction can help before feeds when the nose is blocked.

Avoid honey, over-the-counter cough and cold products, essential oils, and steam that could burn. Do not prop a sleeping baby with pillows; use the usual flat, clear sleep surface. Contact a paediatrician or GP the same day if hoarseness persists, feeding falls, fever develops, or the cough is worsening.

Get emergency help if the baby has blue or grey lips, pauses in breathing, severe difficulty breathing, marked chest or neck pulling, is floppy or hard to wake, or cannot feed because of breathlessness. A harsh breathing-in noise while calm, drooling with trouble swallowing, or rapid deterioration also needs urgent assessment.

Seek prompt medical care if wet nappies become much less frequent, the mouth is dry, the baby repeatedly vomits feeds, or you are worried that the baby is becoming dehydrated. Record a short audio or video of an intermittent cough or breathing sound only if doing so does not delay care.

Tell the clinician when the voice changed, how the cough sounds, whether symptoms worsen during feeds or sleep, and how many wet nappies the baby is producing compared with usual. Ask whether the breathing suggests upper-airway swelling and what changes should trigger reassessment overnight.
