Bleeding after sex at 4 weeks pregnant

A small smear that settles may not signal a pregnancy problem. Bleeding that becomes heavy, recurs, or occurs with pain needs assessment because its source cannot be identified from colour or timing alone.

Pregnancy increases blood flow to the cervix, making its surface easier to irritate during penetration. Cervical inflammation, a cervical polyp, vaginal irritation, or an infection such as chlamydia can also lead to contact bleeding.

Bleeding after sex does not show whether the pregnancy is developing normally. Miscarriage and ectopic pregnancy can present with early-pregnancy bleeding, although the bleeding may only happen to be noticed after sex rather than be caused by it.

Tell your maternity service or obstetrician about the episode, including how much blood you saw, its colour, whether it stopped, and whether you have cramps, one-sided pain, discharge, urinary symptoms, or dizziness. Use a pad to monitor further loss.

Pause penetrative sex until the bleeding has stopped and you have received advice, especially if it returns after intercourse. A clinician may suggest an examination, infection testing, pregnancy hormone blood tests, or ultrasound depending on your symptoms and pregnancy dates.

Seek emergency help for heavy or increasing bleeding, severe abdominal or pelvic pain, pain focused on one side, shoulder-tip pain, fainting, marked dizziness, breathlessness, or profound weakness. These warning signs require prompt evaluation even if bleeding began after sex.

Arrange timely clinical review if spotting keeps returning, or if you have fever, unpleasant-smelling discharge, pain during sex, or burning when passing urine. Infection testing and treatment may be needed rather than simply waiting for the bleeding to disappear.

You might ask whether your symptoms point to cervical irritation, whether infection tests or an examination are appropriate, and when ultrasound can locate the pregnancy. Also clarify when it is reasonable to resume sex and exactly which changes warrant emergency care.
