No period four weeks after miscarriage: could you be pregnant?

Take a home pregnancy test if you have had unprotected sex since the loss. The result must be interpreted in context because hormone from the previous pregnancy can remain detectable for a time, particularly if no negative test was documented afterwards.

The menstrual cycle restarts as pregnancy hormones fall, and the timing varies with how far the pregnancy had progressed and how the miscarriage was managed. Emotional stress, illness, breastfeeding, previous irregular cycles, and retained pregnancy tissue can also delay bleeding.

A positive test may indicate a new pregnancy, hormone remaining from the loss, or pregnancy tissue still in the uterus. One test cannot always distinguish these, so a clinician may arrange repeat hormone blood tests, ultrasound, or both.

If your home test is positive, contact your obstetric or gynaecology service and tell them when the miscarriage occurred, how it was treated, whether a follow-up test became negative, and when unprotected sex occurred. Do not estimate a new pregnancy's dates from the miscarriage bleeding.

If the test is negative and you otherwise feel well, record the result and monitor for your period. Arrange a non-urgent review if menstruation remains absent, especially if your cycles were previously regular, so pregnancy and other causes of missed periods can be assessed.

Seek urgent assessment for one-sided or severe pelvic pain, shoulder-tip pain, fainting, marked dizziness, or weakness, whether or not a home pregnancy test is positive. A new pregnancy can be ectopic, and remaining tissue or infection can also cause complications after miscarriage.

Prompt care is also needed for heavy bleeding, fever, chills, worsening lower abdominal pain, or offensive-smelling discharge. These symptoms are more important than the number of weeks since the loss.

Ask whether the test result could reflect the previous pregnancy, whether hormone levels should be followed, and whether ultrasound is needed. If this is a new pregnancy, ask how it will be dated and when its location can be checked; if you do not want pregnancy now, request contraception that suits your health and preferences.
