Short answer

Extreme fatigue at 36 weeks deserves discussion with your maternity team, particularly when it is sudden, worsening, or disabling.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Key message

Late pregnancy places substantial physical demands on the body and sleep may be fragmented, but profound exhaustion can also accompany anaemia, dehydration, infection, thyroid problems, low mood, or a pregnancy complication. Associated symptoms and the change from your normal energy level guide urgency.

Understanding the change in energy

Consider whether the exhaustion improves after rest or makes basic activities such as washing, eating, or walking unusually difficult. Note when it began and whether you also have dizziness, pallor, palpitations, breathlessness, fever, pain, vomiting, diarrhoea, thirst, dark urine, poor sleep, anxiety, or persistent low mood.

Anaemia can present with fatigue, breathlessness, palpitations, or headaches. Dehydration may present with tiredness alongside thirst, dry mouth, reduced urination, or dizziness. These patterns are possibilities for a clinician to assess, not conclusions based on fatigue alone.

What you can do today

Reduce non-essential demands, pace activity, accept practical help, and rest in a comfortable position. Drink regularly unless you have been given a fluid restriction, and choose regular meals containing protein and iron-rich foods. Gentle movement may ease stiffness if you feel steady and your maternity clinician has not advised against it.

Arrange clinical advice rather than self-treating suspected anaemia with extra iron, because testing and the correct dose matter and iron can interact with other medicines. A clinician may review your blood pressure, symptoms, medicines, sleep, emotional wellbeing, and whether blood or urine checks are appropriate.

Get help for these warning signs

Seek emergency help for severe breathing difficulty, chest pain, fainting, confusion, blue or grey lips, one-sided leg swelling with pain, or sudden weakness affecting one side of the body. Do not drive yourself if you feel faint or cannot stay alert.

Call your maternity unit promptly if exhaustion comes with reduced fetal movement, vaginal bleeding, leaking fluid, regular painful contractions, severe headache, vision changes, sudden swelling, persistent upper abdominal pain, fever, repeated vomiting, or inability to drink. New extreme fatigue without those signs still merits timely advice, especially if rest does not restore basic function.

Details to raise at your review

Ask whether your symptoms suggest a need to check for anaemia, dehydration, infection, thyroid dysfunction, or another pregnancy-related issue. Explain how fatigue affects daily function, sleep, food and fluid intake, mood, breathing, heartbeat, and fetal movement, and list every medicine and supplement you take.

Questions people ask

Is extreme fatigue expected near the end of pregnancy?

Energy can fall as sleep and movement become harder, but extreme, sudden, or progressively worsening fatigue should not simply be dismissed as a normal part of pregnancy.

Could anaemia cause this level of tiredness?

Anaemia commonly presents with fatigue and may also cause breathlessness, palpitations, headaches, or pallor. A blood test is used to assess it; symptoms alone cannot confirm the diagnosis.

Should I exercise when I feel exhausted?

Choose rest if you feel faint, unwell, breathless, or unsteady. If there are no warning signs, light movement may be reasonable within the activity guidance provided by your maternity clinician.
