The short answer

Sleep disruption, the physical work of pregnancy, and daily demands may drain energy. More intense exhaustion can also accompany anaemia, dehydration, infection, thyroid problems, low mood, or a pregnancy complication, so a change from your usual pattern matters.

Why energy can drop

Later pregnancy can make comfortable sleep difficult because of fetal movement, heartburn, urinary frequency, leg discomfort, or breathlessness when lying flat. Anaemia commonly presents with fatigue and may add dizziness, paleness, headache, breathlessness, or a pounding heartbeat. Dehydration may bring thirst, dry mouth, darker urine, and weakness.

A clinician will want to know when the exhaustion started, whether rest restores any energy, and whether you have bleeding, fever, pain, vomiting, breathing changes, swelling, headache, visual symptoms, low mood, or a change in fetal movement.

Protecting your energy

Prioritise sleep and brief daytime rests, share strenuous tasks, and pace activity with pauses before you become depleted. Drink regularly, eat varied meals with iron-containing foods, and use pillows to support a comfortable side-sleeping position. Gentle movement can ease stiffness if your maternity team has not advised restrictions.

Contact your maternity team if fatigue is stopping normal self-care or is notably different from earlier in pregnancy. They may review your blood pressure, blood count, nutrition, sleep, mental wellbeing, medicines, and other symptoms. Do not increase iron or use energy supplements without individual advice.

Urgent maternity warning signs

Contact maternity services urgently for reduced or changed fetal movement, vaginal bleeding, leaking fluid, regular painful tightenings, severe abdominal pain, a severe or persistent headache, visual disturbance, sudden swelling of the face or hands, or pain beneath the ribs. Do not wait for a routine visit when these occur.

Call emergency services for chest pain, severe trouble breathing, collapse, confusion, coughing blood, or one-sided leg swelling with pain. Fever, inability to keep fluids down, very little urine, or profound weakness also needs prompt assessment.

Planning the clinical review

Ask whether your fatigue warrants checks for anaemia, blood-pressure complications, infection, thyroid function, or sleep problems. Clarify which iron preparation to use if testing shows a need, what side effects to expect, and whom to call after hours if your symptoms change.

How can I tell ordinary tiredness from extreme fatigue?

Extreme fatigue may feel out of proportion to your activity, appear suddenly, worsen quickly, or prevent eating, washing, working, or staying awake safely. Report that change to your maternity team.

Could low iron cause this exhaustion?

Anaemia can present with marked tiredness, but symptoms do not identify the cause by themselves. A blood test can help a clinician decide whether iron treatment is needed.

Should I rest all day?

Rest is useful, but complete inactivity is not routinely necessary when you otherwise feel well. Balance sleep with gentle movement and follow any activity limits given for your pregnancy.
