Not pregnant three months after miscarriage

Conception does not happen in every cycle. The useful next step depends on whether periods have resumed, how regularly you are trying, age and fertility history, and whether recovery from the miscarriage was complete.

Ovulation can return before the first period, while later cycles may initially be different in length or flow. A negative pregnancy test therefore does not by itself point to a fertility problem. Stress, illness, weight change, thyroid conditions and irregular ovulation can also alter cycle timing.

If pregnancy tissue remained or pregnancy hormone has not cleared, bleeding or tests may stay abnormal and further assessment may be required. Previous medical or surgical management does not automatically prevent a future pregnancy, but complications and individual diagnoses can change the follow-up plan.

If you feel physically and emotionally ready and have not been advised to wait, regular sex through the cycle avoids relying on a single predicted ovulation day. Take a pre-pregnancy folic acid supplement as advised locally, avoid smoking and alcohol, and check that regular medicines are suitable for pregnancy.

Track the first day of each period and note unusually heavy bleeding, pelvic pain or persistently positive pregnancy tests. Arrange a routine appointment sooner if cycles have not returned, are very irregular, or you already had fertility concerns. The clinician may review ovulation, medical history and whether imaging or blood tests are appropriate.

Seek urgent care for severe or one-sided pelvic pain, fainting, shoulder-tip pain, heavy bleeding, fever, offensive discharge or feeling acutely unwell. If a pregnancy test becomes positive and pain or bleeding develops, prompt assessment is needed to confirm where the pregnancy is located.

Book non-urgent review for absent periods, continuing positive tests after the loss, persistent pelvic discomfort or distress that is disrupting sleep and daily life. Fertility assessment timing is personalised, so earlier advice is reasonable when cycles are absent, age is a concern or there is a known reproductive condition.

Ask whether follow-up confirmed that the miscarriage was complete, whether your cycle pattern suggests ovulation, and which prenatal supplement and medicine changes apply to you. Discuss when fertility tests would be appropriate and what support is available if trying again is bringing back grief or anxiety.
