Bleeding after sex at 11 weeks pregnant

Pregnancy increases blood flow to cervical tissue, which can make it bleed after contact. An examination may be needed because bleeding can also arise from infection, a cervical growth, the vagina or the pregnancy itself.

Note whether the blood is pink, red or brown; whether it appears only on wiping or continues; and whether there are clots, cramps, one-sided pain, discharge, fever, dizziness or urinary symptoms. The timing after sex is useful information but does not establish the source by itself.

Chlamydia may present with bleeding after sex, discharge or pain, though it may cause no symptoms. Cervical dysplasia and cervical cancer can also present with bleeding after sex, but this symptom alone does not show that either condition is present. The clinician will use your history and examination to decide which tests are appropriate.

Use a pad or liner to observe the amount and contact your maternity, obstetric or early-pregnancy service. Tell them your gestation, blood colour and amount, whether bleeding is ongoing, and whether pain or other symptoms are present. Follow their advice about examination, ultrasound or infection testing.

Avoid further vaginal sex until the bleeding has stopped and you have received guidance, particularly if it recurs. Do not insert a tampon to monitor blood loss. If your blood group requires special consideration after bleeding, the treating service will advise what applies to your stage of pregnancy and circumstances.

Seek emergency assessment for heavy or rapidly increasing bleeding, severe abdominal pain, strong one-sided pain, shoulder-tip pain, fainting, marked dizziness, breathing difficulty or feeling very unwell. Obtain prompt clinical advice for fever, unpleasant-smelling discharge, persistent bleeding, clots or worsening cramps. Urgency is based on these warning signs, not on whether bleeding followed sex.

Ask where the bleeding appears to come from, whether the pregnancy needs an ultrasound, and whether cervical or infection testing is indicated. Confirm when sexual activity can resume, what recurrence should prompt another review, and which out-of-hours service to use if the bleeding becomes heavier.
