Short answer

Swollen hands and feet on waking can result from overnight fluid shifts, inflammation, medication effects, or a wider health problem.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Observe
Symmetry, warmth, redness, pain, and skin indentation
Record
Morning symptoms alongside evening changes and recent medicines

How to interpret it

Look for symmetry, warmth, redness, pain, stiffness, and whether pressing the skin leaves a temporary indentation. Swelling that improves after moving around may reflect fluid redistribution, while persistent stiffness in several small joints can be an inflammatory pattern. Rheumatoid arthritis commonly presents with hand and foot joint swelling and morning stiffness, but these symptoms do not establish that diagnosis. Recent travel, salty meals, heat, pregnancy, new medicines, or reduced movement may also be relevant.

Swelling with breathlessness, chest discomfort, faintness, or a sudden marked change needs urgent assessment. Tell a clinician if the swelling is new, steadily worsening, or accompanied by reduced urine, unusual fatigue, or facial swelling.

Steps that may help

Remove tight jewellery and footwear, walk gently if comfortable, and elevate the hands or feet while resting. Avoid prolonged sitting or standing and keep a note of morning and evening changes. Do not use compression garments if one limb is painful, cold, numb, or changing colour until a clinician has advised you. Review new medicines with a pharmacist or doctor rather than stopping treatment yourself.

If a joint is hot or painful, rest it and use a wrapped cool pack briefly. Arrange an appointment if the pattern repeats or does not settle, bringing photos and a list of medicines, illnesses, and recent changes in activity.

When to seek care

Seek emergency help for swelling with difficulty breathing, chest pain, confusion, fainting, or swelling of the face or tongue. Seek same-day care for a hot, red, very painful joint, fever, one-sided calf swelling, or rapidly worsening swelling. A routine review is appropriate for recurring morning swelling, ongoing joint stiffness, or swelling that interferes with walking or hand use.

Questions for your doctor

Ask whether you need an examination, blood tests, urine tests, or imaging; which causes fit the distribution of swelling; and whether any current medicine should be reviewed. Ask what changes should prompt urgent help.

Questions people ask

Is morning swelling always serious?

No. Temporary fluid shifts and other mild triggers can contribute, but recurring, worsening, or widespread swelling should be reviewed.

Should I drink less water?

Do not restrict fluids without medical advice. A clinician can guide fluid and salt changes after considering your health.
