Why is a quadriceps tendon graft used in ACL reconstruction?

The ACL helps limit unwanted forward movement and rotation of the shin relative to the thigh. During reconstruction, the surgeon removes or works around the damaged ligament and places a tendon graft through tunnels in the bones, securing it so the body can incorporate it as a new stabilising structure. Quadriceps tendon tissue comes from above the kneecap and can be prepared with or without a small bone block, depending on the surgical plan.

This choice may suit someone whose anatomy, previous surgery, sport demands or other considerations make a different graft less suitable. Every graft has trade-offs, including soreness at the harvest site, rehabilitation needs and possible weakness; the surgeon matches the option to the knee and the person.

Before surgery, discuss graft options, smoking, medicines, work, driving and the sport you hope to resume. After surgery, follow the staged rehabilitation plan: early priorities commonly include controlling swelling, restoring knee movement and activating the thigh muscles, followed by strength, balance and activity-specific training. Progress should be based on examination rather than a calendar alone.

After reconstruction, contact the surgical team for increasing wound redness, drainage, fever, calf pain or swelling, or pain that is becoming harder to control. Seek urgent help for sudden breathlessness or chest pain. A locked knee, a new giving-way episode or a sudden loss of movement also deserves prompt review.
