Why does spondylolisthesis cause radiating leg pain?

A vertebra that has moved relative to its neighbour can alter the shape of the nerve opening or spinal canal. The affected nerve may be pressed, stretched, or irritated as the back moves. Because that nerve carries sensation from the lower back or buttock into the leg, the resulting pain can travel along its route. It may feel sharp, burning, electric, or like a deep ache.

Radiating pain may be accompanied by tingling, numbness, or weakness, depending on the nerve involved. Sitting, standing, bending, or walking can change symptoms. The location of pain alone cannot identify the cause; a clinician considers the examination and the full symptom pattern.

Arrange an assessment if pain travelling into the leg persists, limits activity, or is joined by altered sensation. The clinician may check strength, reflexes, sensation, balance, spinal movement, and walking. A personalised plan may include physiotherapy for trunk and hip control, activity pacing, posture guidance, and appropriate pain relief. Keep activities within a tolerable range and avoid repeatedly provoking sharp or spreading pain.

Seek urgent care for new or progressive weakness, difficulty lifting the foot, loss of bladder or bowel control, numbness around the inner thighs or genitals, or walking that becomes unsafe. Prompt review is also needed after a significant injury or when severe symptoms occur with fever.
