Why does patellofemoral pain syndrome cause knee pain?

Short answer
Most people with Patellofemoral Pain Syndrome notice knee pain because repeated loading can irritate tissues around the kneecap and alter how the joint moves.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common location
- Front of the knee or around the kneecap
- Typical triggers
- Stairs, squats, running, and prolonged bent-knee sitting
What is happening in the knee?
The kneecap glides in a shallow groove at the front of the thigh bone. Bending the knee for stairs, squats, running, or rising from a chair increases pressure between these surfaces. If the surrounding muscles, hip control, or activity load do not manage that force well, sensitive tissues around the joint may become painful.
Pain is often felt at the front of the knee or behind the kneecap. It may be an ache, pressure, or sharper discomfort after repeated bending, and prolonged sitting with the knee bent can also bring it on. Knee Pain is a very common presentation of Patellofemoral Pain Syndrome.
What may help
Temporarily reduce the movements that reliably flare the pain, while keeping comfortable activity. A gradual strengthening programme can target the thigh, hip, and buttock muscles so the leg controls kneecap loading more effectively. A physiotherapist can adjust exercises, running volume, footwear, and movement technique to your symptoms.
Cold therapy after activity may ease soreness for some people. Increase one activity at a time and allow symptoms to settle between sessions rather than repeatedly pushing through a flare.
When to seek care
Arrange an assessment if pain persists, limits walking or sleep, repeatedly returns, or does not improve with sensible activity adjustment. Seek prompt medical care after a significant injury, or if the knee is very swollen, hot, locked, visibly deformed, or difficult to bear weight on.
Questions for your doctor
Ask which movements are aggravating the kneecap, whether hip or thigh weakness is contributing, and how quickly to progress strengthening. You can also ask whether another knee problem needs to be considered if swelling, locking, or instability is present.

Questions people ask
Why does sitting sometimes hurt?
Keeping the knee bent for a long time increases contact pressure at the patellofemoral joint, which can make sensitive tissues ache.
Can I keep exercising?
Often, comfortable low-impact activity is possible while high-load bending is reduced and strength is rebuilt gradually.
