Why does panic disorder cause shortness of breath?

Short answer
Panic disorder can cause shortness of breath because a panic attack activates the body's alarm response, which can make breathing fast, shallow, and difficult to control.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common experience
- Shortness of breath can occur during a panic attack.
- Breathing pattern
- Fast or shallow breathing can intensify air hunger and light-headedness.
- Important caution
- New or severe breathlessness needs medical assessment.
The short answer
Shortness of Breath is a very common presentation of Panic Disorder. The sensation can be frightening even when the body is taking in air; it may feel like you cannot complete a satisfying breath or that your chest is too tight to breathe normally.
What happens during a panic attack
During panic, the nervous system prepares for danger. Your heart may race, chest and shoulder muscles can tense, and breathing may speed up. Breathing quickly can lower carbon dioxide in the blood, which may add tingling, light-headedness, chest discomfort, or a sense that more air is needed.
These physical sensations can then intensify fear and keep the episode going. Panic disorder involves recurrent attacks and ongoing concern or behavioural changes related to future attacks, rather than a single isolated spell of anxiety.
What you can do in the moment
If this pattern has been assessed as panic, pause where you are and loosen anything tight around your chest or neck. Let the out-breath be gentle and slightly longer than the in-breath, without forcing deep breaths. Focus on a nearby object, name what you can see and feel, and remind yourself that the surge will pass.
Arrange a routine medical appointment if attacks recur. A clinician can check for other contributors to breathlessness and discuss treatment such as talking therapy, breathing and anxiety-management skills, or medicines when appropriate. Avoid using someone else's medication to manage an attack.
When breathlessness needs urgent care
Seek urgent medical help for new, severe, worsening, or unexplained shortness of breath, especially with chest pressure or pain, fainting, blue or grey lips, confusion, coughing blood, swelling in one leg, or symptoms after a serious allergic reaction. Do not assume a new breathing problem is panic.
Get prompt assessment when breathlessness occurs with fever, wheezing, a new cough, reduced ability to do usual activities, or a known heart or lung condition. A panic episode and a physical illness can occur at the same time.
Questions to take to your appointment
Ask whether your symptoms fit panic disorder or whether tests are needed for heart, lung, thyroid, anaemia, or medication-related causes. You can also ask which treatment approach suits your attack pattern, what to practise between episodes, and which warning signs should override your usual panic plan.

Questions people ask
Can panic make you feel as if you are not getting enough air?
Yes. Fast, shallow breathing and tense chest muscles can create air hunger, even though the feeling itself does not show what is causing the breathlessness.
Should I breathe into a paper bag during a panic attack?
Do not use a paper bag. It can be unsafe if breathlessness has another cause. Slow, unforced breathing and urgent assessment for red-flag symptoms are safer responses.
Can a clinician help if my breathing symptoms keep returning?
Yes. A clinician can assess recurrent episodes, rule out relevant physical causes, and help you build a treatment and self-management plan for panic disorder.
