Why does ovarian torsion cause pelvic pain?

How twisting creates pain
An ovary is held in place by supporting tissues that also carry blood vessels. If the ovary rotates around these attachments, the twist can first obstruct venous drainage and cause swelling. Continued twisting may restrict arterial flow. Stretching, swelling, and irritation of nearby pelvic tissues send pain signals that may be sharp, cramping, or constant.
