What this means
Myofascial pain syndrome involves painful, tight areas within a muscle and the fascia around it. In the lower back, hip, buttock, or abdominal muscles, this sensitivity may be felt as a deep ache, pressure, or a sharper spot. The painful area may not be the only place involved: a trigger point can send discomfort into nearby tissue, especially when the muscle is loaded or held in one position.
Protective tightening can then develop. Moving differently to avoid pain places extra work on neighbouring muscles, while prolonged sitting, lifting, reduced activity, or repetitive posture may keep the area irritable. This pattern does not by itself identify the cause of back pain, so an examination helps distinguish muscle-related pain from nerve, joint, or other problems.
What you can do
Keep gentle daily movement within a tolerable range rather than staying in bed. Short walks, frequent position changes, and gradual return to ordinary activities can prevent stiffness. Heat may help a guarded muscle relax; use a comfortable temperature and protect your skin.
A physiotherapist can assess movement, muscle tenderness, strength, and contributing habits, then tailor mobility and strengthening exercises. Ask which activities should be modified temporarily and how to progress them. Do not force a painful stretch or repeatedly press a tender spot if symptoms worsen.
When to seek care
Arrange an assessment if pain keeps returning, limits sleep or work, or does not settle with sensible self-care. Seek urgent medical help for new loss of bladder or bowel control, numbness around the groin, marked leg weakness, fever with severe back pain, or pain after a serious injury.
