Why does inflammatory breast cancer cause breast skin changes?

Lymph vessels normally drain extra fluid from breast tissue. When inflammatory breast cancer involves or obstructs these vessels, fluid can build up in the skin and breast. The breast may become swollen, red, warm, heavier or tender. The skin can look thickened or dimpled, sometimes resembling orange peel. The nipple may flatten or turn inward, and one breast may change shape or size quickly.

The word inflammatory describes the appearance and the immune response around the breast; it does not mean a routine infection is the cause. Redness and warmth can also occur with infection, injury or other breast conditions, so the appearance requires clinical assessment rather than self-diagnosis.

Contact a breast clinic or clinician promptly for new, rapidly developing redness, swelling, warmth, dimpling, nipple change or breast enlargement. Record when the change began and whether pain, fever, discharge or a lump is present. A clinician may arrange breast imaging and a tissue sample when needed. Do not delay assessment while waiting to see whether the skin settles on its own.

Seek same-day medical advice for rapid breast change, spreading redness, severe pain, fever, feeling faint or becoming very unwell. Go to emergency care for breathing difficulty, collapse or severe systemic symptoms. Urgency is based on the speed and severity of the change, not on assuming what the diagnosis will be.

Ask what conditions could explain the skin change, which imaging is appropriate, whether a biopsy is needed, and when results and follow-up will be available. Mention any recent breastfeeding, infection, injury or treatment.
