Why does heart failure cause shortness of breath?

The left side of the heart normally moves blood onward from the lungs. When it cannot do this effectively, pressure can rise behind it. Fluid may collect in lung tissue, so the lungs become less able to expand and transfer oxygen comfortably. Shortness of Breath is a very common presentation of Heart Failure.

Breathlessness may first appear during exertion, then with ordinary movement, or when lying flat. Waking suddenly short of breath, needing more pillows, or feeling unable to finish a sentence are important details to report. Lung infections, asthma, blood clots and other conditions can also make breathing difficult.

Contact your heart-care clinician if your usual breathing pattern changes. Explain what activity triggers it, whether it happens at rest or overnight, and whether you have cough, leg swelling, rapid heartbeat or reduced appetite. Sit upright if lying flat worsens breathing, and follow your existing medicine, fluid and salt plan rather than making unplanned changes.

Prepare a current list of medicines and note any missed doses, recent illness, or new over-the-counter products before you speak with a clinician.

Call emergency services for severe breathing difficulty, chest pressure or pain, fainting, blue or grey lips, confusion, or pink frothy sputum. Seek urgent assessment if breathlessness develops at rest, escalates quickly, or is accompanied by marked new swelling, a racing heartbeat, or inability to lie down.
