Why does compartment syndrome cause calf pain?

How pressure causes pain
The calf muscles, nerves and blood vessels sit inside sections surrounded by firm connective tissue. When swelling or bleeding develops inside one of these spaces, the tissue has little room to expand. Pressure can make the calf feel tight, full or unusually firm, and pain may become severe or out of proportion to the visible injury. Pain can increase when the affected muscles are gently stretched. Nerve compression may add tingling, numbness or weakness. Acute compartment syndrome can progress quickly after a fracture, crush injury, tight cast or intense swelling, so its warning pattern is different from ordinary post-exercise soreness.
What to do now
Severe or rapidly increasing calf pain after an injury needs urgent medical assessment. Do not massage the area, exercise through the pain or attempt to loosen a cast yourself. Keep the limb in the position advised by emergency staff and tell them about increasing tightness, altered sensation or weakness. Clinicians assess the examination findings and may measure compartment pressure. If pressure threatens muscle or nerve tissue, emergency treatment may be needed to release the tight covering.
When to seek emergency care
Go to an emergency department immediately for escalating calf pain, a tense swollen calf, pain with passive ankle or toe movement, new numbness, weakness or difficulty moving the foot. A foot that becomes pale or cold is also an emergency sign. Do not wait for skin colour changes, because serious pressure can be present before circulation looks obviously different.
