Why does basal cell carcinoma cause a skin ulcer?

Basal cell carcinoma begins in cells in the outer layer of skin. As the cancer grows, it can break through the surface and form a sore, crust, or shallow crater. Repeated crusting, bleeding after minor contact, and reopening after seeming to improve can occur because the abnormal tissue continues to interfere with normal repair. Skin Ulcer is a common presentation of Basal Cell Carcinoma, but an ulcer has many possible causes, including injury, infection, inflammation, and pressure. Its appearance alone cannot establish the diagnosis.

A clinician may examine the sore with magnification and take a small skin sample, called a biopsy, if basal cell carcinoma is suspected. The sample is examined to identify the cell type and guide treatment.

Arrange a dermatology assessment for an ulcer that persists, repeatedly scabs, bleeds with little contact, or fails to close. Until it is assessed, rinse gently with clean water, pat dry, and cover it with a clean non-stick dressing if clothing rubs against it. Avoid picking the crust, applying irritating products, or using leftover prescription creams. Bring a list of medicines and note when the sore first appeared, whether it has changed, and any episodes of bleeding or discharge.

Seek prompt medical care if the ulcer is rapidly enlarging, has persistent bleeding, develops spreading redness or warmth, produces pus, or is accompanied by increasing pain, fever, or feeling unwell. An ulcer near the eye, nose, ear, or mouth also deserves timely assessment because these areas contain structures that may be affected as a lesion grows. Urgent help is appropriate for bleeding that does not stop after firm, continuous pressure with clean gauze.

Ask whether the ulcer needs a biopsy, what other causes are being considered, and how its location and size affect treatment choices. You can also ask how to protect the area while it heals, which changes should be reported, and when the treated skin should be checked again.
