Why does anaemia in cancer cause shortness of breath?

To compensate, the heart and lungs may work harder, particularly when walking, climbing stairs, or doing personal care. Shortness of Breath is a common presentation of Anaemia in Cancer, but breathlessness also has other possible causes that need assessment.

Red blood cells use haemoglobin to transport oxygen from the lungs around the body. When their number or haemoglobin level is low, muscles and organs receive less oxygen with each heartbeat. This can produce a fast heartbeat, tiredness, light-headedness, or a need to pause for breath.

Cancer itself, bleeding, reduced nutrition, inflammation, and some treatments can contribute to anaemia. The pattern and timing of symptoms help the clinical team decide what tests are needed.

Tell your cancer team about new or worsening breathlessness, including whether it happens at rest or only with activity. Pace tasks, sit down for washing or dressing if needed, and avoid pushing through dizziness. A clinician may check blood counts and look for other contributors such as infection, fluid around the lungs, or a heart problem.

Seek urgent medical help for sudden or severe trouble breathing, chest pain, fainting, blue or grey lips, confusion, coughing up blood, or breathlessness that prevents you from speaking in full sentences. Contact your care team promptly for a clear worsening from your usual level, fever, or new swelling in a leg.

Ask what may be contributing to your anaemia, whether a blood test is needed now, and which symptoms should trigger same-day contact. If treatment is discussed, ask about its purpose, expected monitoring, and how it fits with your cancer plan.
