White bumps under the skin on the face

The feel and distribution provide useful clues. Smooth, firm, pale bumps may fit milia; rough bumps around hair follicles can occur with keratosis pilaris. Itchy bumps after heat or sweating may fit heat rash, while tender follicle-centred spots can occur with folliculitis. A persistent, flat-topped or violaceous rash deserves assessment because lichen planus can have a different appearance. These patterns overlap, so a photograph and examination are more reliable than trying to diagnose from colour alone.

Wash gently, avoid picking, and choose non-comedogenic moisturiser and sunscreen if these suit your skin. Reduce friction from masks, helmets, or rough towels, and rinse sweat from the face when practical. Do not layer several new active products at once. Record where the bumps appear, whether they itch or hurt, and whether heat, shaving, or a product precedes them. A clinician may recommend targeted care; an oral antihistamine can sometimes relieve itch when medically suitable, while cryotherapy is reserved for selected diagnosed lesions rather than unknown bumps.

Book a dermatology review if the bumps persist, spread, recur, scar, or fail to respond to gentle care. Seek prompt help for rapidly increasing swelling, severe pain, pus, fever, facial swelling near the eye, breathing difficulty, or a widespread blistering reaction. Urgency depends on these warning features, not simply on how many bumps are present.

Ask which pattern is most consistent with the examination, whether a medication or product could be contributing, and which ingredients to avoid. Ask whether testing or a procedure is needed, and how to protect the skin while it settles.
