Whirlpool sign and ovarian torsion

The ovary is held by tissues containing vessels. If these tissues twist, Doppler imaging may show the vessels coiling around one another, producing the whirlpool sign. Other ultrasound features can include an enlarged ovary, follicles pushed toward the edge, or a visible cyst. The sign is not a diagnosis in isolation. Ovarian torsion commonly presents with pelvic pain, and nausea or vomiting can accompany the pain; the pattern and severity matter.

If this sign was reported, ask the imaging or treating team how urgently you need review. Keep the report available and describe the exact start, location, and changes in your pain. A gynaecology team may recommend observation in selected situations, but suspected torsion can require urgent laparoscopy to untwist the ovary and protect its blood supply. If a cyst contributed, laparoscopic ovarian cystectomy may be considered when appropriate.

Go to an emergency department for sudden severe pelvic or lower abdominal pain, vomiting, faintness, fever, or pain that is rapidly worsening. Seek help again if symptoms return after easing. A symptom-free interval does not reliably rule out intermittent twisting. Arrange transport rather than driving if you feel weak, dizzy, or distressed.

Ask what the radiologist saw, whether the ovary is enlarged, and whether a cyst or other finding is present. Ask whether your blood flow was assessed, when a gynaecology review should occur, and what symptoms should trigger immediate return. Discuss fertility-preserving options if surgery is being considered.
