Whirlpool ovarian torsion ultrasound: what it means

Doppler ultrasound tracks movement in blood vessels. In ovarian torsion, the ovary and its supporting tissues can twist, creating a coiled pattern in the vessels that may look like a whirlpool. The scan may also show an enlarged ovary, reduced blood flow, or a cyst, although findings can vary. An ultrasound result does not by itself confirm or exclude torsion. A pelvic pain presentation is common in ovarian torsion, often with sudden, one-sided pain and nausea or vomiting.

Contact the clinician or emergency service that arranged the scan and follow the stated plan immediately. Do not eat or drink if a treating team tells you to prepare for possible surgery. Bring the report, medication list, and details about when the pain began. Treatment may involve urgent keyhole surgery to untwist the ovary and assess its blood supply; a cyst may sometimes be treated during the same operation. A laparoscopic ovarian cystectomy is one procedure your team may discuss when a cyst is involved.

Seek emergency care for sudden or worsening pelvic pain, repeated vomiting, faintness, marked weakness, fever, or a swollen and tender abdomen. These warning symptoms need assessment even if an earlier ultrasound was reassuring, because torsion can be intermittent and ultrasound cannot replace clinical review. Do not drive yourself if you feel faint or severely unwell.

Ask whether the scan showed a twisted vascular pattern, whether blood flow was seen, and how the result fits your symptoms. Clarify how quickly you need review, whether you should avoid food and drink, and what operation or observation would be considered. Ask what follow-up is needed for any ovarian cyst.
