When should you worry about vulval skin changes if you have lichen sclerosus?

Vulval Skin Changes is a very common presentation of Lichen Sclerosus. The condition can make skin pale, thin, fragile, itchy, or sore, and it may gradually alter the shape of the vulval tissues. Scratching and friction can cause cracks or bleeding. A new thickened area, persistent ulcer, lump, or change that does not settle with prescribed treatment needs examination because lichen sclerosus can coexist with other problems.

Use your prescribed treatment exactly as directed and attend planned follow-up, even when symptoms improve. Wash gently, pat dry, avoid fragranced products, and use bland protective emollients if recommended. Record new areas, bleeding, pain, itching, and response to treatment. Do not stop treatment suddenly or self-treat a new lesion with steroid, antifungal, or antiseptic products without advice.

Contact your clinician promptly for a new lump, an ulcer or sore that persists, unexplained bleeding, increasing pain, a change in colour or texture, or treatment that is no longer controlling symptoms. Seek urgent help for heavy bleeding, severe swelling, fever, or difficulty passing urine. These findings need assessment; they do not prove a serious diagnosis on their own.

Ask whether the current appearance is consistent with lichen sclerosus, whether a persistent area needs sampling, and how often follow-up is appropriate. Ask how to apply treatment, protect fragile skin, manage fissures, and recognise changes that should be reported sooner.
