When to worry about vulval pain with vaginal thrush

Vaginal thrush can inflame the vulval skin, causing itching, burning, redness, swelling and small cracks. Urination or sex may sting when skin is irritated. These symptoms can overlap with bacterial vaginosis, sexually transmitted infections, dermatitis and other causes, so a first episode or an uncertain diagnosis should be checked rather than repeatedly self-treated.

Use an appropriate antifungal treatment only as directed by a clinician, pharmacist or the product instructions. Keep the area dry, avoid scented products and douching, and choose loose breathable underwear. Do not scratch or use leftover antibiotics, which do not treat thrush. Avoid sex if it is painful. If symptoms are not improving as expected, arrange review instead of adding more products.

Seek medical advice for severe or worsening pain, fever, pelvic pain, sores or blisters, foul-smelling or unusual discharge, bleeding, or pain when passing urine that is more than surface stinging. Arrange review if symptoms persist after treatment, return repeatedly, occur during pregnancy, or are accompanied by symptoms in a sexual partner. Urgent help is needed if you feel seriously unwell.
