When to worry about a skin ulcer with pyoderma gangrenosum

Pyoderma gangrenosum can cause a painful ulcer with an inflamed or dusky edge. The wound may worsen after minor trauma or an invasive procedure, a response sometimes called pathergy. This is why a new wound should be assessed in the context of your diagnosis rather than managed by repeated injury or aggressive cleaning.

Follow the dressing plan from your specialist and handle the wound gently. Avoid picking, scrubbing, or arranging procedures on the ulcer without discussing them with the treating team. Note its pain, edge, drainage, and spread. Take prescribed medicines as directed and tell the team if treatment is difficult to tolerate.

Seek prompt advice for rapid enlargement, a new dusky border, escalating pain, heavy drainage, bleeding, fever, or increasing warmth and redness. Urgent assessment is needed for uncontrolled bleeding, severe illness, or pain that suddenly becomes difficult to manage. A suspected infection may need separate treatment, but do not self-treat with leftover antibiotics.

Ask how often the ulcer should be reviewed, which dressing is suitable, what changes indicate a flare, and how to plan safely around biopsies or procedures. Share your current medicines and any bowel, joint, or eye symptoms that appeared with the skin problem.
