When to worry about thickened skin with lichen simplex chronicus

Thickened Skin is a very common presentation of Lichen Simplex Chronicus. Repeated rubbing or scratching makes the outer skin layer leathery and more marked. Itching can continue the cycle, especially during sleep or when the area is dry. The appearance may take time to settle after scratching is controlled.

Use the treatment prescribed for the itch or inflammation exactly as directed. Keep nails short, apply a plain moisturiser, and use a cool compress instead of scratching. Soft clothing can reduce friction. Do not pick at scale or use strong antiseptics unless a clinician has advised them; irritation can keep the itch cycle active.

Arrange a dermatology review if the patch is enlarging, bleeding, oozing, ulcerating, developing a new lump, or becoming distinctly tender. Seek prompt care for spreading redness, warmth, pus, fever, or rapidly worsening pain, which can signal infection or another problem. A sore that persists despite treatment also deserves reassessment.

Ask whether the diagnosis still fits, what is driving the itch, how long to use each treatment, and how to protect the site at night. Mention any new bleeding, ulceration, discharge, pain, or change in texture.
