When to worry about a skin rash with psoriasis

Psoriasis can produce well-defined, thickened patches with scale, often on the elbows, knees, scalp, or other areas. Flares may follow stress, illness, skin injury, or a treatment change. Cracking and scratching can damage the skin barrier, allowing tenderness, oozing, crusting, or warmth to develop. New joint stiffness, swelling, or pain matters because psoriasis can affect joints as well as skin.

Continue prescribed treatment as directed and use a bland moisturiser to soften scale and reduce cracking. Avoid picking plaques, harsh scrubs, and unadvised combination creams. Photograph a flare in consistent lighting and note whether it followed an infection, injury, or medication change. A clinician can adjust treatment when the rash is expanding, disrupting sleep, or returning quickly after treatment.

Contact a clinician promptly for rapidly spreading redness, extensive peeling, pus, increasing warmth, fever, or feeling acutely unwell. Arrange review for a new rash that differs from your usual psoriasis, persistent bleeding cracks, or eye symptoms. Seek urgent care for breathing difficulty, sudden swelling, severe widespread redness, or a hot, swollen, painful joint that limits movement.

Ask whether this is a psoriasis flare or a different skin problem, how to protect cracked areas, and whether your treatment needs changing. Tell the clinician about joint pain, recent illness, new medicines, and products applied to the rash.
