When to worry about a skin rash with atopic dermatitis

Atopic dermatitis commonly causes dry, itchy, inflamed patches that can crack or thicken after repeated scratching. A flare may affect the face, hands, elbows, knees, or other areas. Oozing, yellow crusts, increasing tenderness, or a rash that becomes hot can signal a secondary infection rather than an ordinary flare. Clusters of painful blisters, rapidly changing skin, or sores around the eyes need urgent assessment because infections and other skin conditions can resemble eczema.

Use the moisturiser and prescribed treatment plan you were given, and keep nails short to reduce skin damage from scratching. Wash gently with lukewarm water, pat dry, and avoid fragranced products or any newly introduced irritant. Do not pick crusts or apply an antibiotic, steroid, or antiseptic product that was not recommended for this episode. Take a clear photo and note when the change began, especially if the rash followed a new product, illness, or medicine.

Arrange medical advice promptly if the rash is worsening despite your usual plan, disrupting sleep, covering new areas, or showing drainage, crusting, warmth, swelling, or pain. Seek urgent help for fever with a rapidly spreading rash, facial or eye swelling, breathing difficulty, extensive blistering, or marked illness. A clinician can examine the skin and decide whether treatment for infection or a different condition is needed.
