When to worry about skin blisters with chickenpox

Chickenpox spots can change into itchy, fluid-filled blisters before crusting. Scratching can break the surface and let bacteria enter. Increasing tenderness, warmth, swelling, cloudy drainage, or redness that moves outward is more concerning than itch alone. Blisters near the eyes, inside the mouth, or in the genital area can need specific assessment, especially when eating, drinking, or urinating becomes difficult.

Keep the skin clean and gently pat it dry. Avoid popping blisters, scratching, or sharing towels and clothing. Trim nails and use a cool compress for itch. Contact a clinician promptly if a blistered area is worsening, if fever returns after settling, or if the person seems unusually unwell. Chickenpox can spread, so follow local medical advice about staying away from people at higher risk.

Get urgent help for breathing difficulty, confusion, severe drowsiness, repeated vomiting, a stiff neck, trouble walking, or a rapidly spreading painful rash. Seek same-day advice for eye involvement, extensive skin soreness, pus, or a high or persistent fever. Infants, pregnant people, and anyone with a weakened immune system should contact a clinician early after chickenpox symptoms appear.

Ask whether the blister changes suggest a bacterial infection, whether treatment is needed, and how to protect household members. Mention when the rash began, whether new blisters are still appearing, any fever, and medicines or immune conditions.
