When to Worry About Shoulder Pain With Shoulder Osteoarthritis

Short answer
Shoulder Pain is a common presentation of Shoulder Osteoarthritis, but sudden change, injury, severe stiffness, or signs of illness should be assessed.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common symptoms
- Aching, stiffness, grinding, and reduced reach can occur.
- Seek prompt care
- Watch for injury, sudden weakness, heat, redness, fever, or chest symptoms.
What may be happening
Shoulder osteoarthritis can cause aching, stiffness, grinding, and reduced reach as the joint surfaces and surrounding tissues become irritated. Symptoms may make dressing, grooming, or reaching a shelf harder. A sudden sharp change needs assessment, particularly after a fall or when the shoulder becomes hot, swollen, or unusually weak.
What you can do
Keep the joint moving gently within a tolerable range and divide demanding tasks into smaller periods. Avoid forcing a stiff shoulder through a painful stretch or carrying loads that worsen symptoms. Record changes in movement, sleep, swelling, temperature, and hand or arm sensation. A clinician can assess the joint and discuss activity changes, physiotherapy, and suitable pain-relief options.
When to seek care
Get urgent assessment after an injury if the shoulder changes shape, the arm cannot be lifted, or pain is severe and sudden. Seek urgent help for chest pressure, breathlessness, faintness, sweating, or pain extending to the jaw or arm. Contact a clinician promptly for fever, a red or hot joint, new numbness, rapidly worsening stiffness, or pain severe at rest.
What to ask
Ask whether your symptoms fit the shoulder joint or another source, which movements to practise, and how to manage flares. Ask how strength, sensation, and movement will be monitored and which changes should lead to an earlier appointment.

Questions people ask
Is grinding alone an emergency?
Grinding without sudden weakness, injury, or illness is not usually an emergency, but persistent pain or lost function merits review.
Should a stiff shoulder be stretched forcefully?
No. Gentle, tolerable movement is preferable; forcing a painful stretch can aggravate symptoms.
