When to worry about shoulder pain with a rotator cuff tear

Short answer
Shoulder Pain is a common presentation of Rotator Cuff Tear, but urgent review is needed after a new injury with sudden weakness, inability to raise the arm, deformity, or severe swelling.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common functional clue
- Painful weakness when lifting or reaching
- Urgent change
- Sudden inability to raise the arm after injury
Pain and strength changes
A rotator cuff tear can cause pain with lifting, reaching, or lying on the affected side, along with weakness or a catching sensation. Some tears develop gradually, while others follow a forceful event. A sudden change in strength matters because it may indicate a new or larger tear, although pain itself can also temporarily inhibit movement.
What to do next
Pause heavy overhead work and movements that cause sharp pain. Keep the elbow, wrist, and shoulder moving gently within a tolerable range unless you have been given an immobilisation plan. Book assessment when weakness affects dressing, grooming, lifting, or sleep, or when symptoms are not improving. Examination helps guide physiotherapy, imaging, injections, or surgical discussion when appropriate.
When to seek urgent help
Seek urgent care after a fall, sudden pull, or dislocation-like event if you cannot raise the arm, the shoulder looks abnormal, swelling is rapidly increasing, or the hand is numb, pale, or cold. Prompt review is also needed for fever with redness and warmth, or for worsening pain that is not explained by activity.
Questions to ask
Ask which tendon is involved, how strength compares between shoulders, whether imaging is needed, and which movements are safe during recovery. Discuss how your work, sleep, and daily tasks should shape the treatment plan.

Questions people ask
Can a rotator cuff tear cause night pain?
Yes. Pain may worsen when lying on the affected side and can disturb sleep, particularly after daytime shoulder use.
Does weakness need medical assessment?
New, worsening, or function-limiting weakness should be assessed, especially when it began after an injury or prevents arm elevation.
