When to worry about shoulder pain with cervical radiculopathy

Cervical radiculopathy happens when a nerve root in the neck is irritated or compressed. Pain may travel from the neck into the shoulder, arm, or hand, with tingling, numbness, or weakness in a particular pattern. Neck position can change the symptoms. A clinician checks reflexes, sensation, strength, and movement to assess the nerve and exclude a shoulder-only cause.

Keep gently active and change positions regularly rather than staying in bed. Avoid heavy lifting or repeated neck movements that clearly worsen arm symptoms. Arrange a clinical review if pain persists, sleep is affected, or tingling and weakness interfere with work or grip. Record which fingers feel numb and whether the symptoms are spreading or becoming stronger.

Get emergency help for rapidly worsening weakness, clumsy hands, balance or walking changes, or new difficulty controlling bladder or bowel function. Seek prompt assessment after significant trauma, or with fever, severe unrelenting neck pain, or symptoms affecting both arms or legs. Chest pressure, breathlessness, sweating, or faintness also needs emergency evaluation rather than being attributed to the neck.

Ask which nerve signs are present, whether neck or shoulder imaging is appropriate, which activities to modify, and which exercises are suitable. Explain any weakness, dropping objects, altered balance, or change in sensation.
