When to worry about shortness of breath with bronchiectasis

Shortness of Breath is a common presentation of Bronchiectasis. Damaged, widened airways can collect mucus, making coughing and clearing secretions harder. Breathlessness may increase during a chest infection, often alongside more sputum, a change in sputum colour, fever, wheeze, or fatigue. A sudden change can also have another cause, including a clot or heart problem, so do not rely on your baseline symptoms to judge a serious episode.

Stop and rest in an upright position. Use your airway-clearance technique, inhalers, oxygen, or rescue plan only as prescribed. Contact your respiratory team or clinician promptly if sputum increases, becomes harder to clear, or changes noticeably with worsening breathlessness. Keep a note of temperature, sputum changes, chest pain, and any blood. Drink fluids if your clinician has not restricted them, and avoid smoke or other airway irritants.

Call emergency services for severe or rapidly worsening breathing difficulty, blue or grey lips, collapse, confusion, severe chest pain, or coughing up a large amount of blood. Seek immediate help if you cannot speak comfortably because of breathlessness. Sit upright while waiting and do not drive yourself. If blood in sputum is new or recurring but not heavy, contact your clinician urgently for advice.

Ask for a written flare-up plan covering sputum changes, antibiotics, airway clearance, and oxygen use. Discuss how to recognise your usual baseline, when to arrange a sputum sample, and which amount of blood requires emergency help.
