When to worry about a rash with fever during a drug rash

A drug rash can arise after starting, stopping, or changing a medicine, and the fever may reflect an immune reaction. Mild cases may look like scattered pink or red spots and cause itching. More serious reactions can affect the whole body and may progress quickly. Skin tenderness, dusky patches, blisters, peeling, or sores on moist surfaces such as the lips, mouth, eyes, or genitals are warning features. The rash pattern and the timing of every medicine, supplement, and recent infection help a clinician judge the cause.

Contact the prescribing clinician promptly and give the exact medicine name, dose, start date, and time the fever began. Do not take another dose until a clinician gives you a safe plan, unless it is a medicine that must not be stopped suddenly. Keep a list of all products and photograph changes in the rash. Avoid applying new creams or taking leftover medicines that could hide progression.

Seek emergency care now for trouble breathing or swallowing, faintness, marked facial or tongue swelling, rapidly spreading blistering, skin peeling, severe skin pain, confusion, or inability to drink. Go urgently as well when fever occurs with eye pain or vision change, mouth sores that prevent fluids, or genital sores. These signs need examination and supportive care without waiting for the rash to settle.
