When to worry about rash with fever in chickenpox

Chickenpox commonly causes an itchy rash that changes from red spots to fluid-filled blisters and then scabs. New spots may appear while older ones are healing, and fever, tiredness, or reduced appetite can occur. Other illnesses can also cause rash with fever, so a clinician may need to confirm the diagnosis, particularly after vaccination or when the pattern is unusual.

Avoid scratching, keep fingernails short, and use cool baths or clinician-approved itch relief. Keep blisters clean and avoid sharing towels, clothing, or personal items. Stay away from pregnant people, newborns, and people with weakened immunity until a healthcare professional confirms that you are no longer contagious. Do not give aspirin to a child with suspected chickenpox.

Contact a clinician promptly if fever is high or persistent, the person is drinking poorly, or spots become increasingly red, hot, swollen, painful, or filled with pus. Seek urgent help for breathing difficulty, severe drowsiness, confusion, trouble walking, repeated vomiting, stiff neck, severe headache, or eye pain or vision changes. Get advice early if the patient is pregnant, very young, or immunocompromised.

Ask whether the illness is chickenpox, how to control itching safely, when school or work can resume, and whether antiviral treatment should be considered. Mention vaccination status, pregnancy, immune conditions, and recent contact with vulnerable people.
