When to worry about a persistent mouth sore with oral leukoplakia

Oral leukoplakia is a persistent white or grey area in the mouth that cannot be explained by simple wiping. A sore may occur in the same area or alongside a rough, thickened, red, or mixed-colour patch. Irritation from tobacco, teeth, dentures, or infection can affect the mouth too; a clinician may need to examine the tissue closely and sometimes sample it to clarify the diagnosis.

Contact the clinician or dentist monitoring your oral leukoplakia if a sore develops, lasts, or changes. Photographing the area for your own record can help you describe progression, but it does not replace examination. Avoid tobacco, reduce irritants such as alcohol and very hot foods, keep dentures clean and well fitted, and do not scrape or pick the patch.

Seek prompt review for bleeding, increasing thickness, a red area within or beside a white patch, a firm lump, persistent pain, numbness, loose teeth, or a neck swelling. Seek urgent help if swelling interferes with breathing or swallowing, or if bleeding will not stop. A change in a monitored patch should be reported even if it is not painful.

Ask whether the sore and leukoplakia need a fresh examination, whether a biopsy or dental referral is suitable, and how often follow-up should occur.
