When to Worry About Pelvic Pain With Interstitial Cystitis

Interstitial cystitis can cause pressure, tenderness, or aching in the bladder and pelvic area. Symptoms may change with bladder filling and may ease after urination, but a new pattern should not automatically be attributed to your usual condition. Urinary infection, kidney problems, bowel conditions, menstrual causes, or other pelvic conditions can also produce pain.

Notice where the pain is, when it began, whether it is constant or comes in waves, and whether urination, bleeding, fever, vomiting, or vaginal symptoms occur with it. These details help a clinician separate a flare from another problem.

For familiar, mild symptoms, follow the flare plan provided by your clinician, drink fluids according to your usual advice, and avoid personal triggers such as foods or drinks that have worsened symptoms before. Keep a brief symptom record rather than repeatedly checking or pressing the painful area.

Arrange a medical review if pain keeps returning, interferes with sleep or daily activities, or does not settle with your usual plan. Do not start leftover antibiotics, because treatment depends on the cause and may require urine testing.

Seek urgent medical care for severe or rapidly worsening pain, fainting, confusion, repeated vomiting, a rigid or very tender abdomen, fever or chills, blood in the urine, inability to pass urine, or pain with possible pregnancy. These symptoms can signal a problem that needs examination rather than home monitoring.
