When to worry about pelvic pain with adenomyosis

Adenomyosis occurs when tissue similar to the uterine lining grows into the uterine muscle. The uterus may become tender, and cramping can spread through the pelvis or lower back. Pain may be strongest during periods but can also linger between them or occur during sex. Heavy bleeding and fatigue may occur alongside pain. A pelvic pain flare can have more than one cause, so a symptom pattern needs assessment rather than self-diagnosis.

Contact your clinician if pain is becoming more frequent, lasts longer, or limits movement, work, intimacy, or sleep. Record timing, severity, bleeding, bowel or bladder changes, and what eases or worsens the discomfort. Continue an agreed treatment plan and ask before using medicines if you are pregnant, trying to conceive, or have another health condition. A review can help separate adenomyosis symptoms from infection, pregnancy-related pain, or another pelvic condition.

Seek urgent care for sudden severe pain, fainting, fever with pelvic tenderness, repeated vomiting, heavy bleeding with weakness, or a rigid or very swollen abdomen. If pregnancy is possible, do not wait for pain to settle before seeking advice, especially with one-sided pain or bleeding. Call emergency services if you collapse or feel dangerously unwell.
