When to worry about pain during sex with endometriosis

Endometriosis-related pain may feel deep in the pelvis, especially with deeper penetration, and can continue after sex. It may vary with the menstrual cycle and occur alongside period pain, pelvic aching, bowel symptoms, or urinary discomfort.

Pain can also reflect vaginal dryness, pelvic floor muscle tension, infection, inflammation, or another pelvic condition. Pain during sex does not establish one diagnosis, even when endometriosis is already known.

Pause or change sexual activity when it hurts. Write down where the pain is, whether it occurs at entry or deeper penetration, how long it lasts afterward, and any bleeding, discharge, fever, or urinary symptoms.

Book a review if pain is recurring, affects intimacy, or makes you avoid sex. A clinician can discuss examination, pelvic floor care, lubrication, pain treatment, and whether your endometriosis plan needs adjustment.

Seek urgent help for severe or rapidly worsening pelvic pain, heavy bleeding, fainting, fever, repeated vomiting, or pain with a possible pregnancy. Arrange prompt assessment for new bleeding after sex, unusual discharge with pain, or pain that remains intense after stopping.

Ask whether the pain suggests deep endometriosis, pelvic floor involvement, dryness, or infection, and whether an examination is needed. Ask which treatments can reduce pain during intimacy and what symptoms should trigger urgent care.
