When to worry about ovulation pain with endometriosis

Endometriosis can make pelvic tissues sensitive around hormonal changes. Pain may be one-sided, cramping, or felt deep in the pelvis, and it can last beyond the brief discomfort some people notice around ovulation. A change from your usual pattern matters more than the calendar timing alone.

Pain can also have other causes, including an ovarian cyst, infection, bowel or bladder conditions, or pregnancy-related problems. The symptom does not identify its cause by itself.

Record when the pain starts, where you feel it, how long it lasts, and whether bleeding, discharge, fever, nausea, bowel symptoms, or urinary symptoms occur with it. Note whether prescribed endometriosis treatment changes the pattern.

Arrange a routine appointment if pain repeatedly interrupts work, sleep, movement, sex, or daily activities, or if self-care no longer helps. A clinician may review your history, examine you, and decide whether imaging or pregnancy testing is appropriate.

Seek urgent medical help for sudden severe pelvic pain, fainting, marked dizziness, heavy bleeding, fever with pelvic pain, repeated vomiting, a rigid or swollen abdomen, or pain with a possible pregnancy. These features can signal a problem that needs prompt examination.
