When to worry about night pain with a rotator cuff tear

Short answer
Pain That Wakes You at Night is a common presentation of Rotator Cuff Tear. Arrange assessment when the pain persists, strength declines, or you cannot use the shoulder normally.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common presentation
- Night pain can occur with a rotator cuff tear.
- Strength change
- Increasing weakness deserves assessment.
What night pain can mean
A rotator cuff tear can make the shoulder painful when you lie on the affected side or move the arm during sleep. Pain may be felt at the outer shoulder and can occur with weakness, clicking, or difficulty reaching overhead. Night pain does not by itself show how large a tear is. Your clinician may consider the injury, shoulder movement, strength, and how symptoms began, particularly if there was a fall, a sudden pull, or a lifting injury.
What to do at home
Avoid movements that sharply increase pain, but keep the elbow, wrist, and hand moving gently so they do not become stiff. Try sleeping on your back or on the opposite side with the affected arm supported by a pillow. Use only pain relief that is suitable for you and follow the label or medical advice. Book a review if sleep remains disrupted, daily tasks are becoming harder, or weakness is increasing.
When to seek urgent care
Get urgent assessment after a new injury if you suddenly cannot lift the arm, the shoulder looks deformed, or there is severe swelling, numbness, or loss of feeling in the hand. Seek prompt care for fever, a hot red joint, or unexplained worsening pain. These signs may point to a fracture, nerve involvement, or another shoulder problem requiring examination rather than home treatment.
Questions for your doctor
Ask which movements are safe, whether physiotherapy is appropriate, and whether imaging would clarify the tendon injury. Discuss sleep positioning, suitable pain relief, expected recovery milestones, and which changes should bring your appointment forward.
