When to worry about a neck lump with a thyroid nodule

A thyroid nodule is a growth within the thyroid and may be felt in the lower front of the neck. Some nodules cause pressure, a cough, hoarseness, or difficulty swallowing; many cause no noticeable symptoms. A neck lump can also arise from lymph nodes, muscles, skin, or infection.

Changes such as faster growth, firmness, reduced movement, or nearby swelling are reasons to arrange review. These features do not determine the diagnosis by themselves; an examination and appropriate testing are needed.

Arrange an appointment if the lump is new, enlarging, persistent, or different from a previously assessed nodule. Note its location, any change in voice or swallowing, and whether you have pain, fever, or recent infection.

A clinician may feel the thyroid and neck, review thyroid blood tests, and arrange an ultrasound. If the ultrasound shows features needing clarification, a needle sample may be discussed. Keep any scan reports for comparison.

Seek urgent help for difficulty breathing, inability to swallow liquids, rapidly increasing swelling, sudden severe voice change, faintness, or confusion. Fever with a hot, red, very painful swelling may also need prompt assessment.

Ask whether the lump matches the known nodule, whether an ultrasound is needed, and what changes should prompt a faster review. Ask when thyroid blood tests or a needle sample would be considered.
