When to worry about discoloured nails and nail psoriasis

Discoloured Nails is a common presentation of Nail Psoriasis. The nail may show yellow-red patches, pits, ridges, thickening, crumbling, or separation from the nail bed. Changes can affect fingernails, toenails, or both, and skin or joint symptoms may occur at the same time.

A new colour pattern is not automatically part of psoriasis. Fungal infection, injury, bacterial growth, and pigment disorders can look similar. A clinician can compare the nail with your usual pattern and decide whether testing or a change in treatment is appropriate.

Keep nails short, file rough edges gently, protect hands from repeated wet work, and wear comfortable shoes that do not press on affected toenails. Avoid cutting cuticles, picking under the nail, or applying artificial coverings over a lifting nail. Moisturise the surrounding skin if it is dry or cracked.

Continue prescribed psoriasis treatment as directed and tell your clinician if the nail changes despite treatment. Photograph the nail when symptoms flare so changes can be compared at follow-up.

Seek prompt assessment for warmth, swelling, pus, spreading redness, marked tenderness, or a nail that lifts quickly. Arrange review when one nail changes in a way unlike the others, a dark stripe develops, or nail pain limits daily activities. Fever or feeling unwell needs urgent medical advice.

Ask whether the change matches nail psoriasis or suggests infection, whether a sample is useful, and how to protect the nail bed. Mention joint stiffness, scalp or skin flares, recent injury, previous fungal infection, and all treatments you use.
