When weakness needs attention

Mild weakness that stays stable may be part of the tear and the pain-related reduction in shoulder use. A clinician can check whether the weakness comes from the tendon injury, pain inhibition, stiffness, or another problem affecting the shoulder or nerves.

What the weakness may mean

The rotator cuff helps centre the upper-arm bone and move the shoulder in different directions. A tear can make lifting the arm, reaching overhead, rotating the arm, or holding an object away from the body difficult. Pain may also make you stop using the shoulder fully, which can leave the surrounding muscles less conditioned.

Muscle weakness is a common presentation of Rotator Cuff Tear. Weakness that is new, clearly progressing, or much greater than the pain warrants a fresh examination. Numbness, tingling, or weakness extending into the hand may point to nerve involvement rather than the tendon alone.

What to do now

Arrange an appointment with an orthopaedic clinician or physiotherapist if strength is declining, daily tasks are becoming harder, or the shoulder is not improving with the plan you were given. Note when the weakness began, which movements fail, whether pain comes first, and whether there was a fall, lifting incident, or other trigger.

Until you are assessed, avoid movements that sharply increase pain or force the arm overhead. Keep the shoulder gently moving within a comfortable range if advised, and follow the exercise or activity instructions already provided. Do not test strength repeatedly against heavy resistance.

When to seek urgent care

Seek prompt medical care after a new injury if you suddenly cannot lift or rotate the arm, the shoulder looks deformed, or severe pain and weakness begin together. Urgent assessment is also appropriate for a hot, red, swollen shoulder with fever, or for new numbness or marked weakness in the arm or hand.

If weakness develops suddenly with facial drooping, speech difficulty, confusion, or weakness in another part of the body, treat it as an emergency and contact local emergency services. These features are not explained by an isolated rotator cuff tear.
