When to worry about lower back pain with a vertebral compression fracture

Short answer
Lower Back Pain is a common presentation of Vertebral Compression Fracture. Seek prompt care if pain is suddenly worse, follows a new injury, affects movement, or comes with weakness, numbness, or bladder and bowel changes.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance

- New neurological symptoms or pain after injury need urgent assessment.
What the symptoms can mean
A vertebral compression fracture happens when a spinal bone weakens and partially collapses. Pain is often focused in the back and may increase when standing, walking, coughing, or changing position. A change in posture or reduced height can develop over time. Severe pain, pain after a fall, or symptoms travelling into the legs may indicate a new problem or pressure near spinal nerves.
What to do now
Contact a healthcare professional if pain is not settling, prevents sleep or basic movement, or returns after improvement. Avoid heavy lifting, forceful twisting, and unadvised bed rest. Move carefully, keep frequently used items within easy reach, and follow any brace or activity instructions you have received. Assessment may include examination and imaging, followed by a plan for pain control, safe activity, and bone health.
When to seek urgent care
Get urgent help for new leg weakness, spreading numbness, numbness around the groin, difficulty passing urine, or loss of bowel control. Attend promptly after a significant fall or other impact, particularly when pain is severe or movement becomes difficult. Fever or feeling acutely unwell with new back pain should also be assessed without delay.
Questions for your doctor
Ask whether the fracture appears stable, what movements and lifting limits apply, and whether a brace or physiotherapy is appropriate. Discuss warning signs of nerve pressure, whether bone-strength evaluation is needed, and how to make your home safer while recovery progresses.

Questions people ask
Should I stay in bed?
Prolonged bed rest can cause weakness and stiffness. Follow your clinician's advice about safe, supported movement instead.
Why can pain return after it improves?
A new strain, another fracture, or changes around the healing bone can cause renewed pain, so a persistent change should be reviewed.
