When to worry about leg numbness and tingling with sciatica

Short answer
Leg numbness and tingling is a very common presentation of sciatica, but new weakness, loss of bladder or bowel control, numbness around the groin, or rapidly worsening symptoms need urgent medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common pattern
- Tingling may travel from the lower back or buttock into the leg.
- Urgent warning
- Bladder, bowel, groin-sensation, or major strength changes need urgent care.
What the sensation can mean
Sciatica involves irritation or pressure affecting a nerve root in the lower back. Sensations may travel from the back or buttock into the thigh, calf, or foot, and can feel burning, buzzing, prickling, or reduced sensation. Symptoms may change with sitting, coughing, bending, or walking. The pattern and strength of the leg matter more than the sensation alone.
What to do now
Keep gently mobile if movement is comfortable, change position regularly, and avoid lifting or twisting that clearly increases the leg symptoms. Note when the tingling began, where it travels, whether it is constant, and any change in walking or strength. Arrange a clinical review if it persists, interferes with sleep or daily activities, or is getting worse.
When to seek care
Seek emergency help for numbness around the inner thighs or genitals, difficulty starting or controlling urination, loss of bowel control, or severe symptoms affecting both legs. Obtain prompt assessment for foot weakness, repeated tripping, a foot that slaps while walking, or rapidly increasing numbness. These signs can indicate significant nerve dysfunction.
Questions for your doctor
Ask which nerve pattern your examination suggests, whether your strength and reflexes are changing, which activities to modify, and what symptoms should trigger urgent reassessment. Ask when physiotherapy or further tests would be appropriate for your particular pattern.

Questions people ask
Is tingling alone an emergency?
Tingling without weakness or bladder, bowel, or groin-sensation changes is not automatically an emergency, but persistent or worsening symptoms deserve a clinical review.
Can sciatica affect walking?
Pain or altered sensation can change your gait. New difficulty lifting the foot or repeated tripping should be assessed promptly.
