When should you worry about knee swelling if you have acl tear?

Short answer
Knee Swelling is a common presentation of ACL Tear, but urgent assessment is needed when swelling is sudden and severe, the knee cannot bear weight, or the leg is numb, cold, or misshapen.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common pattern
- Swelling and restricted movement after a twisting knee injury
- Urgent concern
- Deformity, inability to bear weight, or a cold, numb, discoloured leg
Why swelling happens
An ACL tear can cause bleeding or irritation inside the knee, particularly after a twisting injury. Swelling may make the knee feel tight and limit bending. Ongoing swelling after activity can also reflect continuing irritation or another injured structure, such as a meniscus. Knee swelling has several possible causes, so its presence alone does not confirm an ACL tear. Read more about ACL Tear or Knee Swelling.
Protect the knee
Stop pivoting, running, jumping, and other activities that make the knee give way. Rest with the leg supported, use a wrapped cold pack for short periods, and follow any brace or crutch instructions you were given. Avoid testing stability repeatedly. Arrange an orthopaedic or physiotherapy assessment to discuss movement, strength, instability, and whether further evaluation is appropriate.
When swelling needs prompt care
Seek urgent care after a major injury if the knee is visibly deformed, you cannot bear weight, or it is stuck in one position. Emergency assessment is also appropriate for a foot or leg that becomes cold, pale, blue, numb, or unusually weak. Get same-day advice for rapidly increasing swelling, severe pain, fever, or a hot red joint, because these findings need examination.
Questions to bring
Ask whether the swelling pattern fits an ACL injury and whether other structures may also be involved. Discuss brace or crutch use, safe weight-bearing, rehabilitation timing, and which instability symptoms should trigger review. Ask how to return to work, exercise, or sport without sudden twisting loads.

Questions people ask
Can swelling return after an ACL tear?
Yes. Activity can irritate the injured knee and bring swelling back, especially when rehabilitation is advanced too quickly.
Should I walk on the knee?
Follow your clinician's weight-bearing advice; use support if walking increases pain, instability, or swelling.
