When to worry about knee pain with knee bursitis

Short answer
Knee Pain is a common presentation of Knee Bursitis, but fever, spreading redness or rapidly increasing swelling should be assessed urgently.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common trigger
- Kneeling or repeated pressure can irritate a knee bursa.
- Urgent warning
- A hot, spreading red area with fever may need urgent assessment.
What knee bursitis can feel like
A bursa is a small fluid-filled sac that helps tissues move smoothly. Irritation around the knee can cause local tenderness, swelling and pain with kneeling, bending or pressure. The swelling may sit over the kneecap or on the inner side of the knee, depending on the bursa involved. Most people with Knee Bursitis notice knee pain, but severe heat, redness, fever or feeling unwell can indicate infection rather than simple irritation.
Comfort measures
Avoid kneeling and direct pressure, and modify activities that repeatedly bend the knee. Rest with the leg supported and apply a cold pack wrapped in fabric for brief intervals. Keep the skin clean and avoid trying to drain the swelling yourself. A clinician can recommend suitable pain relief and check whether footwear, work tasks or an underlying joint problem is contributing.
Warning signs
Seek urgent care if redness spreads, the area becomes increasingly hot, swelling grows quickly, or you develop fever, chills or marked weakness. Prompt assessment is also needed if you cannot bend or bear weight, or if pain follows a significant injury. Book a non-urgent review when swelling persists, returns often, or restricts work, sleep or everyday movement.
Questions for your doctor
Ask which bursa is affected, whether infection needs to be ruled out, and whether fluid testing or imaging is appropriate. Discuss safe activity changes, pain relief, protective padding and how to reduce recurrence at work or during sport.

Can bursitis cause visible swelling?
Yes. Irritation may produce local swelling and tenderness over or beside the knee.
Should I puncture a swollen bursa?
No. Draining it yourself can introduce infection; have persistent or worsening swelling assessed.
